Setting up a Custom Question

Common custom questions include: "Which subdivision do you live in?" or "Can your child swim 25 yards?" NOTE: waivers and anything that charges an extra fee (such as living outside the HOA) are managed in the next section, the Acknowledgement Section. And t-shirt sizes are managed in the "Merchandise" tab. *

Liability and Waiver

Copy and paste your team's or league's liability waiver info for acknowledgement here. You can have as many acknowledgement sections, or custom questions, as you need in your form, simply add more sections if necessary.

Examples of other uses of acknowledgement sections include a Code of Ethics, Media Release, Volunteer Commitment acknowledgement, or even questions that pertain to fees, like residency.
